Only one form of submission is required. Please do not submit multiple ballots.

Submission of ballot forms via email and/or facsimile transmission is not permitted. You must file an originally executed ballot.

Privacy of Information Collected

The information requested on the ballot is being collected for the purpose of considering the approval of the debtors’ plan for reorganization. The information you provide on the ballot will be retained by or on behalf of the U.S. Bankruptcy Court, the debtors and Kroll Restructuring Administration (formerly known as Prime Clerk) for as long as necessary for the purpose of considering the approval of the debtors’ plan for reorganization, as needed to resolve disputes or protect legal rights of the parties to the plan or ballots, or as otherwise required by law.

Some or all of the information you provide on the Ballots may be shared with third parties affiliated with this matter in furtherance of the Plan. Although you may have certain rights relating to the information provided on the ballot under certain laws, applicable law or court order may prohibit the amendment or erasure of such information once it is submitted.
